Plains Medical Center earns grant

Plains Medical Center has been awarded $35,000 by Health Resources and Services Administration (HRSA) to assist in the adoption of a Patient-Centered Medical Home model.

"A Patient-Centered Medical Home is a model of care that strengthens the clinician-patient relationship by replacing episodic care with coordinated care and a long-term healing relationship," according to Morgan Honea, CEO/President for Plains Medical Center. "Each patient has a relationship with a primary care clinician who leads a team that takes collective responsibility for patient care. PMC is excited to be at the forefront of the movement toward more personalized, coordinated, effective and efficient care."

PMC is a Federally Qualified Healthcare Center providing comprehensive primary medical, dental and mental health services to more than 17,000 residents of Lincoln, Kit Carson, Elbert, Adams and Arapahoe Counties. For more information call 719-775-2367
